All tandem riders must sign a Form 115a BPA Tandem Student Medical Information – Declaration. Please note that only forms issued by the British Parachute Association can be used for this purpose; writing a family doctor is not enough. Be sure to notify Click and Jump once you have reached your fundraising goal and give Click and Jump the time to take the right precautions and send a voucher that can hand you over to the parachute center instead of the payment. If you must have indicated a form by a family doctor, we advise you to check with your family doctor before booking, as booking fees cannot be refunded if a family doctor does not sign the medical form. Scan or take your own copy of your form before visiting the drop-down area as soon as you pass it – they keep the original for their recordings and may not be able to provide you with a copy. If your charity has agreed to pay the balance of your fundraiser, you must reach your goal 1 month before your jump date and inform your charities and Click and Jump that this is the case. Not all forms are available to the public here on the UK skydiving website. If the form you are looking for is not listed, please contact us with the number and name of the form you need. As mentioned above, the minimum age for skydiving is 16.
However, when a person is under the age of 18, they must have their parent or legal guardian sign the eligibility section of the form below for the BPA affiliation agreement. In addition, it is imperative that the same parent or legal guardian sign the declaration of fitness to be a witness. After reading BPA 115A, if you doubt your ability to do a tandem jump, print out the form below (115B) and take it to your doctor for advice. Please note that the National Health Service does not provide this advice and may charge you an invoice. You may need to pay for additional tests or recommendations needed to determine your risk. The following forms have been drawn up by the British Parachute Association. To open it, you need a free Adobe Reader program – you can download it here. Please print these forms and complete them (including the control area) and bring them on the day of your jump.
Please note that we cannot accept letters or notes from doctors – this special form must be used. If you change your contact information, you should notify Click and Jump as soon as possible. We sometimes receive reports from some web users who are not able to download pdf forms from the UK skydiving site. This can be a problem with some browsers or pdf readers. If you are unable to download a form, please contact us with the form number you have accepted. It is important that anyone who parachutes is able to do so. Skydive St Andrews must have a fitness statement for every person who jumps with us, in accordance with the requirements of the British Parachute Association. Everyone should check the application form for the tandem paratrooper student (form 115A) to verify medical requirements and authorization to self-report fitness. More information can be found below. The minimum age for skydiving is 16. Anyone who parachutes under the age of 18 must have the written consent of the parents or legal guardians – which must be commented on your membership form – and have the same parent or guardian sign the declaration of fitness to testify.
Please note that people over the age of 70 should contact the instructor`s head before booking. The forms are provided with your information pack, booking confirmation and are available for downloading clickandjump.co.uk/forms. Please take these forms with you on the day of your jump (don`t post them to Click and Jump or drop-down area and don`t give them to someone to take care of you). Without this formula
